Remember to Enclose Coupon with Payment
Please enclose your billing statement coupon anytime you make a payment, along with coupons for each subgroup if you are paying for multiple subgroups. Also, please write your group ID number (the number that begins with a “G”) on your premium check, as well as your group name if it is not already on the check. This will enable us to apply the payments correctly. Thank you for your assistance.
Please send any other documents or correspondence, such as terminations, to the address on the enclosed Enrollment Change Form. For any questions or concerns, your Representative’s contact information is listed on your statement under “Questions?” at the top of page one. When emailing, please include your group name and number in the body of your email.
EFT Payment Option Makes Premium Payment Easier
Thanks to our EFT (Electronic Funds Transfer) payment option for groups, paying your PacificSource health plan premium can be easier.
- EFT means one less check to process, as well as easier reconciliation.
- The regularly scheduled payments are confirmed by email
- No postage and reduced administrative costs add up to significant savings.
It’s easy to sign up, too. Contact your Membership Representative to get started. The EFT form (Group Authorization Agreement for Recurring Electronic Fund Transfers) is available in the
Employers > Forms and Materials section, under Manage Your Group Coverage.

Use the Most Current Enrollment Forms
The updated 2019 Enrollment Applications and other forms are available at in Employers > Forms and Materials area of our website. There you will find the most up-to-date administrative forms for your use.
Please Complete and Return Your COBRA Determination Form
Last month, we mailed COBRA determination forms to employers. If you have already returned your form, thank you! If not, we ask that you please take a moment to complete it and fax it to your Membership Representative at the fax number on the form.
Allow Plenty of Time for Mailed Payments
Holidays and the end of the month are often busy times for post offices. If you mail your bill, please allow ample time for mailing to be sure it arrives to us before the end of your grace period, especially during these peak times.

Access Plan and administration information online 24/7. Use InTouch to:
- Enroll new members and update existing member information
- Access your plan’s contract documents and member benefit materials
- View current census information and enrollment totals
- Print temporary ID cards for one or all employees with the click of a button
- Pay your bill online with eCheck (not available for all groups)

Tobacco Cessation Outreach Calls
We all know it’s difficult to quit tobacco, but it's easier when there is someone who can help. The Quit For Life® Program has helped thousands of people, including PacificSource members, take control of their lives and their health by quitting tobacco. Beginning soon, program representatives will place personal calls to members who have indicated on their health assessment through CaféWell that they use tobacco. Representatives will tell these members about the program and invite them to participate.
